Job Details
Need a basic wordpress site with a sql (mysql is fine) database.
The home page will display a report from data collected by an input form.
Above the report will be a button to submit a new form which feeds into the report.
The report itself will be a simple table with a link that drills down into a second input form.
The forms must support captcha for input.
On submit the user will be provided confirmation of submitting the form online as well as by email.
I will provide access to the server.
I will provide a mock up of what the form will look like (its very simple) there are only about 15 fields to track.
I will provide a mock up of the report. the input form(s) that populate the report. and of the drill down form when clicked from the report.
There will probably be additional work.
I need this completed right away.
